After clicking a New Dog Forms, Evaluations & Notes button, select Dog Commands Evaluation from the record type drop down and click continue.
The commands are sorted alphabetically on the page. Each command field is a number only field, and can only contain the numbers 0,1,2,3 or 4. You can also included brief notes for each command if needed.

Condition
Select the condition under which the evaluation was performed. Standing is the default selection.
